Answering rollcall were Members Andrew Hawkins, Mary Absolon, Matt Giljahn, Megan
D'Orazio, Brenna Smithson, Andrea Leszko, Nick Mattison, Andrea Sullivan, Anoushka
Jha.

Andrea Sullivan made a motion, seconded by Brenna Smithson, to Approve
Meeting Agenda.
Motion passed
4. Approval of Meeting Minutes
4.1. August 12, 2024 Community Health Commission Minutes
Andrew Hawkins made a motion, seconded by Nick Mattison, to approve
August 12, 2024 CHC Meeting Minutes, with the addition of Megan D'Orazio
to the attendance list.

7.1. Thomas Brooks - Diversity, Equity, and Inclusion Manager. - Introduction and
Discussion
Thomas Brooks attended as an introduction and to discuss planned Equity Strategic
Action Plan process and how Community Health Commission can contribute to the
process and development of plan.
7.2. 2025 Community Health Commission Work Plan - Approval and Assignment
Mary Absolon made a motion, seconded by Andrew Hawkins to Approve draft
of 2025 Community Health Commission Work Plan, as discussed.
