tenant of decedent named herein, and that said decedent was also some-
times known as Ava P. Seemann. That Ava I. Seemann died on the 21st
day of Jan. 1945, at the age of 57 years at Minneapolis, State of
Minnesota, with residence at 3111 Newton Ave. North, County of Hennepin,
State of Minnesota. That a duly certified copy of the record of her death

at and prior to death was the owner of an interest as joint tenant in the

Hennepin County, Minnesota, in Book 1914 of .Deeds, page 426, in the
following described property,'to -wit: Lot 5, Block 1, Hamisch's Second
Addition to Minneapolis (homestead). That no part of the above property
was the homestead of decedent unless so specified in the description.
That affiant has disclosed to the Commissioner of Taxation all transfers---- -
of property from the decedent to any beneficiary of which affiant hasr
knowledge or information, which transfers may be subject to Minnesota
inheritance tax. That affiant makes this affidavit and files said
certified copy of record of death as evidence of the death of said joint
tenant and the termination of said joint tenancy and all such estate, tittle
interest and lien as was or is limited upon the life of said decedent.
