Resolution No. 2021-48
Accepting Innovations for Aging, LLC Grant Funding
WHEREAS, Innovations for Aging, LLC, is the managing organization for the Juniper initiative, expanding
evidence-based health promotion (EBHP) classes throughout the State of Minnesota; and
WHEREAS, the City of Edina (Edina Senior Center) shall participate in the Juniper initiative and provide a
series of EBHP classes (Aging Mastery Program) designed to help adults age 50 and older to set goals, engage in
beneficial daily practices and seek peer support to make meaningful changes in their lives and cultivate health and
longevity; and
WHEREAS, Innovations for Aging, LLC will pay the City of Edina $40 per participant who registers/attends a
minimum of one session or $60 for each participant who completes 7 out of 10 sessions of the Aging Mastery
Program; and
WHEREAS, Innovations for Aging, LLC will pay the City of Edina $125 for each completed participant
of the Aging Mastery Program (up to 30 participants) whose participation is covered by BlueCross and
BlueShield of Minnesota’s Health Options health benefit plan; and
WHEREAS, the City of Edina agrees to enter into necessary and required agreement with Innovations for
Aging, LLC with the specific purpose of completing the Aging Mastery Program;
WHEREAS, a request for payment for the number of participants who have completed the program based
on the funding criteria will be made to Innovations for Aging, LLC by the City of Edina no later than 30 days after
completion of the program: and
NOW THEREFORE, BE IT RESOLVED that Edina City Council approves and accepts the Innovations for
Aging, LLC grant agreement and associated grant funding.
