Edina School District; and David Velde I I. APPROVAL OF THE MINUTES: October 29, 1986 Sharon Aadalen asked if there were any correction or comments about the minutes. Hearing none, Sharon Asked for a vote of approval for the October 29, 1987 minutes . All voted aye; minutes were approved. II . REPORTS: j ' I A. STATE CHS ADVISORY COMMITTEE REPORT-Virginia McCollister �. Virginia reported that a bill has been submitted to the Minnesota Legislature requiring licensure of Home Health Care in Minnesota . The proposed bill will provide an inspection program of Home Health providers by the Minnesota Department of Health. This licensure will exclude persons who care for family members. The State Advisory Committee will be establishing a Task Force which will establish guidelines for developing health promotion programs at the local level . The State Committee will also establish a Task Force which will develop guidelines to assist communities in implementing Emergency Medical Services locally. The Administrative Task Force has developed a set of CHS Reporting guidelines to be used by all CHS agencies in reporting annual activities. This program will be piloted in several local agencies prior to adopting the guidelines. Virginia indicated that a bill has been introduced which will recodify all health related State Statutes into one Chapter. The recodification process was initiated by the Department of Health and is expected to pass unopposed . Virginia informed the Committee that the Health Department has recently released a new draft manual for administering Community Health Services in Minnesota. This manual is intended to assist CHS agencies in planning , managing, and evaluating their public health activities. The Health Department has asked for an increase in CHS subsidy of about $1. 3 million for 1988 and the same amount for 1989. The state has not fully funded CHS for several years and this increase will bring the state' s share of funding in line with the enacting legislation. The Department of Health is attempting to increase staffing in the division which responds to complaints about HMO' s . Evidently, during 1986 the number of complaints increased dramatically and the staff was unable to respond appropriately. B. 1987 WORKPLAN: i i David Velde , informed the committee that the City will be developing the 1988-1989 CHS Plan this year and it is expected that the preliminary draft will be completed by June 1987 . A joint Task Force of Richfield, Bloomington and Edina will be meeting in March to begin looking at the community demographics and needs. The Edina CHS Advisory Committee should provide two members to attend the four Task Force meetings . `. The CHS plan will address the delivery of Community Health Nursing Services, Home Health Services, Environmental Health Services, Disease Prevention and Control Services, Emergency Medical Services, Health Education, and Maternal and Child Health Services in Edina. The statewide goals developed by the Minnesota Department of Health will be addressed on the 1988-89 CHS Plan. It is generally believed that the Metro Area has adequately addressed statewide goals in the past and existing programs will adequately address these goals in 1988-89. David Velde informed the Committee that he will be developing specifications for bidding public health nursing services for 1988 and 1989. For the past several contract renewal period the Edina City Council historically has requested the City seek competitive bids for these services from both the public and private sector . The bid specifications will be released in June 1987 and the bids will be evaluated by the Committee in August with a recommendation being forwarded to the Edina City Council in September . It is anticipated that it will take about one month to transfer client records to another agency if Bloomington loses the contract. The record transfer should take place in December '1 "� 1987 to ensure an orderly transition from one provider to the other and to minimize the potential disruption of care to the client. David Velde commented that evaluating the bid proposals is a difficult task due to the different missions of the agencies submitting proposals. The Committee has struggled with the bid proposal evaluation process in the past and the committee should develop a tool to assist us with this function. Sharon Aadalen asked David Velde if he has received a directive from the City Council indicating a desire to bid the nursing services for 1988-89, David Velde indicated that in the past the City Council has expected that these services be offered for competitive bidding . Sharon Aadalen stated that the Committee should discuss this process and come to an understanding within the Committee whether this process should continue and forward a recommendation to the City Council . Sharon stated that when an agency submits a bid for these services they put forth a great deal of effort in developing the bid. When the Advisory Committee reviews the bid we end up spending a great deal of time comparing services which, often cannot be considered as similar. She felt uncomfortable with a process which is supposed to be competitive yet , because of the philosophical difference between public and private providers, the agencies are not totally comparable and therefore not equally likely to be competitive in the process . Holly Branch recalled her position from the last time the City reviewed bids for nursing services. In her judgement she does not believe the City can get the same commitment to a public health objective from a private agency as one can expect from a public agency. She has worked with both public and private agencies in the past and there is a different philosophy of care with a public agency. The public agency will continue with care until the client no longer needs the service. Holly indicated that she has done consulting work with Bloomington, Abbott, Home Health Plus, MMC, United, Mercy and many other agencies relative to home health services. In her dealings with these various agencies she believes that the Bloomington Division of Public Health does a superior job in the areas she is familiar with. She is very impressed with their staff and their whole scope of services . She believes a hospital could not provide comparable services for Edina residents. Holly stated that it would take a number of years for a hospital Home Care service to develop a service comparable to the service provided by Bloomington. Sharon Aadalen stated that Dart of the philosophical differences between private and public agencies is that the public health agencies tend to look at the family-in-community as the unit of care as opposed to the individual . The private '' 1 agency typically will focus on the identified patient in the family. The focus on the community and family is important because the issue of connecting people in need to the available resources in the community is a critical function of a public agency. Private agencies have a tendency to lose sight of this objective because of loss of reimbursement. Another factor Sharon thought should be kept in mind is the continual shifting taking place in the private health care environment. Home Health Plus is an agency that resulted from combining home care services in Edina (Competent Care) , at MMC, and in St. Paul . This is not the only such combination taking place in the private sector and more combining will be taking place in the future. The health care market is in a state of flux which may result in changes in philosophy of the resultant agencies. David Velde indicated that the has been concerned about the possibility of changing nursing service providers every one or two years as it relates to the disruption of service to the Edina residents. Developing a public health care system in a community to the point where it becomes effective takes several years. Just "getting to know the community" is essential for the provider . Changing providers means losing that knowledge. In David' s opinion a long term commitment to a provider is needed to develop an effective system of service delivery in the community. Peggy Kelly asked if there was some services that Fairview Southdale Hospital could not provide the last time they bid on these services. David Velde stated that Fairview Southdale Hospital intended to contract with Hennepin County for Disease Prevention and Control Services. Sharon Aadalen indicated that the Health Promotion component submitted by Fairview Southdale Hospital consisted of utilizing their educational programs in existence or to be developed which were not comparable to the Bloomington proposal . Eileen Cooke recalled that in evaluating the Fairview Southdale proposal the Committee discovered that the professional staff was not comparable to the quality of the professional staff level offered by Bloomington. Jim Hansen asked whether the City was obligated by law to competitively bid for these services. David Velde said the City was not obligated to bid for professional services and that for several years the City negotiated exclusively with Bloomington for nursing services . Virginia McCollister said she concurred with what has been said and that her concern was that a private provider may decide to aggressively seek this contract and submit a bid which is '"1 `� i A i f extremely low and underbid the competition. This situation would create a difficult evaluation problem for the Committee and, if on paper they appeared to meet the specifications, could the committee determine whether this was a responsible bid . David Velde stated that he did not feel he could evaluate the bid proposals fairly. He felt that comparing agencies and the services being offered is an extremely difficult task that requires more resources than he has. Sharon Aadalen brought up another issue about the philosophy of care the private and public agencies have. The private sector will provide whatever services the client wishes as long as these services are reimbursable. The public agencies objective is to return the individual to an independent status , which is sometimes contrary to the individuals wishes. Sharon Aadalen informed the Committee that metropolitan public health nursing agencies have developed an areawide coordinated service system to ensure that residents needing health services will be handled properly in their geographic area of residence . This service system has obtained funding from the Robert Woods Johnson Foundation to implement the program in the metro area . Sharon feels that the City should not ask public agencies to compete against each other for the Edina service area when they are trying to develop collaborative service agreements among themselves. Sharon Aadalen said she has never heard a negative or critical comment about the Bloomington Division of Public Health. She has heard a great deal of comments suggesting that Bloomington has substantial expertise resulting in well planned and well carried out services . Sharon Aadalen asked David Velde if the Committee has the option of advising the City Council to consider negotiating for the contracted services with Bloomington rather then obtaining bids. David Velde said it would be appropriate for the Committee to offer some form of recommendation. Jim Hansen suggested that the Committee proceed with a recommendation to the City Council that we negotiate with Bloomington for contracted services for 1988 and 1989 and that a parameter for a dollar amount be established prior to negotiation. The Committee will meet in two weeks to formulate a recommendation for the City Council . C. TRI CITY TASK FORCE: i I I David Velde advised the Committee that we will be forming a Task Force representing Edina, Bloomington, and Richfield to study the needs in the three communities and develop the needs assessment required in the CHS plan for 1988-89. Each of the i three communities will provide staff and advisory committee members to serve on the Task Force. Two members of the Edina Advisory Committee should serve on this task force. The Task Force will meet 3 to 4 times over the next 3 months. The first meeting will be in early March, 1987. This process has been utilized before and has been a valuable tool in developing the CHS plan . Sharon Aadalen asked for volunteers from the Committee for this Task Force. Tom Orth said he might be able to attend. David Velde will call absent members to see if they could attend these meetings and Peggy Kelly said she might be able to attend if others could not do so . III. OTHER BUSINESS: Eileen Cooke had been asked to discuss the smoking policy in the Edina School District with the CHS Advisory Committee. It has been suggested that the school district prohibit smoking at school between 7: 30 a.m. and 4: 00 p.m. at the very least. Smoking by teachers does not present a good role model for students. The school district has been implementing the Minnesota Clean Indoor Air Act and this policy would carry the non smoking policy one step further . Eileen is asking the committee to offer support for a no smoking policy to be implemented in the Edina School District. After further discussion, Eileen offered a motion that the Committee forward a letter to the school district indicating support for their effort to adopt a smoke free school policy. Tom Orth offered a second to the motion. All members voted aye; motion carried . David Velde will draft a recommendation for Sharon Aadalens' signature. Peggy Kelly asked if the City has been experiencing problems with the water system. She indicated she has had a few calls complaining about rust and scum in the water . David velde said that the City does periodically flush the water mains to control the build up of rust in the system and this procedure does illicit complaints. He indicated that perhaps the Newsletter could include some information about water main flushing to forewarn the community. "1 Peggy Kelly asked for a followup on the City appointment to West Suburban Mediation Service. David said he would follow up on this. Peggy also discussed the annual Volunteers Dinner with the Committee. She indicated that the Committee could recommend someone to be recognized at this event. David Velde said he would provide the information to the Committee at their next meeting . Eileen Cooke offered a motion to adjourn the meeting . Sharon Aadalen seconded the motion. APPROVAL OF THE MINUTES: Sharon Aadalen asked the Committee if there were any corrections or comments for the minutes. Hearing none, Sharon asked for a vote of approval for the minutes of February 25, 1987. All voted aye; minutes were approved. Sharon Aadalen reminded the Committee that at the last meeting the members had approved sending a letter to the Edina School Board supporting a no smoking policy in school buildings . Sharon read the draft letter to the committee. Effie McKerson suggested that this letter advocating no smoking in school should also include the use of tobacco in any form. The letter, in its final form, will include a reference to tobacco use in any form. II . 1988-89 Public Health Nursing Services Recommendation Sharon Aadalen informed the members that in the past the Committee has been concerned about purchasing public health nursing services through a private provider however the Committee has found it difficult to evaluate proposals because of the differences in the content of the proposals . In all cases, the other bidders for these services have subcontracted for some components of the services, notably the Disease Prevention and Control components . It was noted that hospitals are merging their home health services and these services are constantly in a state of flux . F '� i i j Sharon noted that the contract was awarded for two years the last time the bid was awarded. She commented that keeping the public health nursing services contract in the same period as the community health services plan was beneficial for all people involved in these health services . I Dr . Wilder asked how many records must be transferred when j the service provider changes. David Velde said that about three hundred patient records would need to be transferred . Dr . Wilder said that this would be a monumental task and if there isn' t a good reason for changing providers the task of transferring records should be considered as a reason not to change to a different provider. Betty Stinnet asked if there was any indication that the Bloomington Division ofPublic Health was not providing appropriate services in Edina or whether Bloomington was not fulfilling their obligation. David Velde said he was not aware of any problems with the services delivered by Bloomington. Jim Hansen asked if any other organizations had approached the City since the last bidding process indicating their interest in bidding on future contracts. Dave Velde indicated there had been none Effie McKerson indicated that she understood the City Councils position regarding bidding for purchasing materials etc . , but she believes bidding for personal health services has quality issues which may not be identified in a bid process and that this quality issue in the personal public health nursing services contract is very difficult to evaluate in open bidding . Betty Stinnet wondered whether our energies would be better utilized evaluating the present services than evaluating bid proposals. David Velde indicated that beginning in 1986 the City of Edina was able to utilize Federal Maternal and Child Health funding to support existing programs and augment the services delivered to Edina families. These funds are available to public agencies only and if we were to contract with a private provider we may jeopardize these funds or further fragment the delivery system by having another provider of services in the system. Virginia McCollister stated that she didn' t see a strong case to seek bids from other providers for these services . She went on to say that if there were indications that the quality of care provided by Bloomington was questionable or if the costs were escalating beyond reasonableness her response would be different. She feels that the decision to seek bids should be based on evidence that some other provider could do better . '1 i Dr . Wilder suggested that eventually the City may elect to provide these services directly and that this could be explored in future years. However , at the present time he believes that the present system is best for Edina. Jim Hansen offered the following motion : "The Edina CHS Advisory Committee recommends that the City of Edina negotiate a contract with the Bloomington Division of Public Health for personal public health nursing services for 1988 and 1989. The cost for these services should not exceed 40 per year and if an agreement cannot be reached at 40 or less, the City of Edina should seek open bids for these services" Effie McKerson offered a second to the motion. All voted aye; the motion carried . I i David Velde will formulate the recommendation and send a copy to each committee member for their review. This recommendation will be forwarded to the Edina City Council for the April 6, 1986 meeting . i III . OTHER BUSINESS 4 Sharon Aadalen reminded the Committee that -in a previous meeting an evaluation of Public Health Nursing Services had been discussed and that the Committee should concentrate on this I evaluation process during the next two years. Home Health Services would probably be the most likely area •to evaluate first. Other areas would subsequently be evaluated . In evaluating Home Health Services, the provider could give the client a questionnaire which the client could complete and return to the City of Edina . David Velde advised the Committee that the proposed Public Health Act being considered by the state legislature had a statement that all people should have access to the public health care in Minnesota regardless of their place of residence. This statement has a potential impact on City delivered health services if a non resident chooses to utilize the service offered and is unable to pay for the care. David Velde also informed the Committee that the Metropolitan Health Board has recently drafted a report titled "Expanding Access to Health Care for the Low Income Population" . This report suggests a method which could be utilized to provide health care coverage for the working poor in Minnesota. Copies of this report will be distributed to the members and the City Council . IV. All voted aye; motion carried. II. SCHOOL DISTRICT NURSE REPORT: Berit Peterson had asked to be placed on the agenda to update the committee about the school health activities. Berit reported that they are in the process of developing an end of the year report which she will share with the committee when it has been finalized. Berit has been active in several tuberculosis manitoux clinics at the High School as a result of a diagnosed tuberculosis case in a student. Thus far there have been no indications of additional cases as a result of this exposure, however fourteen individuals are receiving additional follow up as a result of positive manitoux tests. The School District is on alert for mumps and measles as a result of an increase in reported cases of these diseases. The school population is in compliance with the expected immunization level for students, however, the immunizations are not totally effective in all individuals. Berit advised the Committee that she is on the Adolescent Pregnancy Prevention Coalition Committee which is a joint effort by Edina, Bloomington, Eden Prairie and Richfield addressing teenage pregnancy. The coalition is involved in teenage pregnancy prevention workshops and addresses the educational need of pregnant teens through the District 287 program which allows the pregnant teen to continue her education, provide parenting classes and day care for the child. Berit also advised the Committee that she has been on the Health Curriculum Committee for the School District and they have just completed their review of all the health curriculum. The Health Curriculum Committee has offered suggestions for changes and when the report is available, she will make it available for this committee. Berit informed the Committee that she has been serving on the Chemical Health Committee. The goals and objectives will be ready by this fall and she will share this with the Committee when they are available. She also stated that the Chemical Health Committee was concerned about the lack of a juvenile police officer being available to the School District. The last Juvenile Officer was promoted and the Police have not replaced him in this capacity. III. ELECTION OF OFFICERS: Effie McKerson offered a motion to nominate Dr. Walter Wilder for chair of the Committee. Betty Stinnett offered a second to the nomination. All voted aye; the motion carried. Jim Hansen offered a motion to nominate Effie McKerson for Vice Chair of the Committee. Betty Stinnett offered a second to the nomination. All voted aye; the motion carried. IV. REPORTS A. Status of Public Health Nursing Contract: David Velde informed the Committee that negotiations with Bloomington for Public Health Nursing Services have been completed. The increase for the contract will be 4 percent each year in 1988 and 1989. David Velde said he will seek City Council approval for the two year contract as soon as possible. B. Environmental Health Countywide Task Force: Jim Hansen reported that the Task Force has recommended that groundwater protection be the key issue for cooperative efforts by the Boards of Health in Hennepin County. Activities will include water well abandonment and public education and awareness. �! i Ii I C. Tri City Planning Task Force: Effie McKerson informed the Committee this Task Force has met six times and have developed the public health nursing programs for the 1988-89 CHS Plan. The program descriptions and activities will be brought back to the Committee during the plan development process which will run between now and August, 1987. D. State CHS Advisory Committee Virginia McCollister reported that the local Public Health Act was passed by both houses and has been sent to the Governor for signing. Virginia said that there will be no increase in CHS subsidy for 1988 or 1989, Emergency Medical Service block grants were funded instead. Virginia advised the Committee that she will be serving on the Health Promotion Task Force which will develop recommendations for Health Promotion Activities for local agencies. They will submit a report to the State CHS Advisory Committee by December, 1987 with their recommendations. Virginia also said the Home Health License Bill was passed and is awaiting signing by the Governor. V. II . 1988-1989 Community Health Services Plan: David Velde informed the Committee that the 1988-89 CHS Plan will be submitted to Hennepin County by September 11, 1987 . The Edina City Council will receive the CHS plan on September 14, 1987 for their action. The resolution approving the plan by the Edina City Council wiil be forwarded to Hennepin County during the week of September 14 , 1987 . Hennepin County will be reviewing the plan in September and the County Board is expected to approve the plan in early October. State statutes requires the review of City CHS plans by the County Board prior to submission of the plan to the Minnesota Department of Health. The CHS plan is compromised of 6 program areas : Environmental Health; Emergency Medical Services; Home Health ; Family Health; Disease Prevention and Control; and Health Promotion. Environmental Health Services and Emergency Medical Services are delivered directly by the City of Edina. Home Health, Family Health, Disease Prevention and Control and Health Promotion Services are delivered through a contract with the Bloomington Division of Public Health. The contract has been awarded to Bloomington for the 1988 and 1989 planning period. -- David Velde summarized the activities which are proposed in each of the Program areas . Environmental Health: Food Service Sanitation Restaurant inspections Food handler education Quarterly newsletters Restaurant operator certification Housing Housing Code complaint investigation and enforcement Community Sanitation Investigation of refuse accumulation complaints Abatement of accumulations of refuse Environmental Pollution Control Water well testing Noise pollution control - Swimming pool inspection Carbon monoxide monitoring in buildings Noxious Weed Control Investigation of tall grass and weed complaints Elimination of noxious weed Animal Control . Investigation and quarantine of animal bites. Loose animal control Public Health Laboratory Provide environmental health laboratory support for Bloomington, Edina and St. Louis Park Chemical and microbiological testing of food and water Emergency Medical Services Provide emergency treatment and transportation to individuals experiencing life threatening injuries or illness Home Health Services i Intermittent home health services to those in need Coordination of services with other community agencies Participate in the Public Health Cooperative ( transcare) to market these services in the area Family Provide affordable prenatal care /---_ L --- - development Provide consultation to day care providers Provide disease and disability services to families Disease Prevention and Control Communicable disease surveillance Focus on AIDS and Lyme disease Testing and referral for Sexually Transmitted Disease Immunization activities Health Promotion Health risk reduction activities Hypertension activities-blood pressure monitoring Accident reduction Behavior modification to promote a health lifestyle Participate in low cost occupational health initiatives for small business Information and education materials I The services delivered by Edina (Environmental Health and Emergency Medical Services) are supported partially by fees . Edina collects about $300,000 in license fees and ambulance fees each year. About 58% of the costs for community health services in Edina are supported by CHS subsidy, grants and fees . The remaining 42% is supported by property tax. Sharon Aadalen offered a motion to approve the 1988-1989 Edina Community Health Services Plan and forward the plan to the Edina City Council for their action. Thomas Orth offered a second to the motion. All voted aye; the motion carried. III. State CHS Advisory Committee Update Virginia McCollister reminded the Committee about the annual Boards of Health meeting which will be held in Alexandria, Minnesota on September 23 , 24 and 25 , 1987 at the Arrowhead resort. The state CHS advisory committee will meet on Wednesday afternoon. Thursday and Friday will be filled with workshops on Public Health topics . Virginia plans on attending the state CHS meeting if she can schedule it. David Velde will be attending the entire meeting and can serve as the alternate if she could not attend the meeting. Virginia informed the Committee that in the latest newsletter called Commentary from the Minnesota Department of Health there was a good article about the Home Health Care Bill passed by the legislature which summarizes the bill quite well . Virginia advised the Committee that the new CHS Planning and Reporting Manual has been released by the Department of Health. wnicn wiii improve the consistency of plan submissions and s "1 reporting among the local agencies . Virginia circulated an MDH Report on Radon in Homes . This report gives background information about Radon, suggests companies who can test for Radon and provides suggestions on what to do if Radon levels are above the action level in the home. There is little information available as of this date about the prevalence of Radon in Minnesota. Virginia said that she is on a Task Force for the state CHS Advisory Committee which is designing Health Promotion guidelines for local health agencies throughout the state. These guidelines are intended to assist local health agencies in designing and evaluating health promotion programs . IV. 1988 Committee Work Plan David Velde reminded the Committee that previously there has been discussion about performing an evaluation of services delivered by Bloomington. It has been suggested that clients using the services should submit an evaluation to Edina thereby avoiding the potential for discomfort on the clients behalf in offering a candid evaluation. After some discussion, a Task Force was selected to develop an evaluation tool to bring back to the full committee. Holly Branch will chair the Task Force. Others on the Task Force are: Sally Tang, Jim Hansen and David Velde who will provide staff support V. Adjournment The meeting adjourned at 9 : 10 p.m.
